Job summary

Health Now Complex Care in Grimsby is seeking experienced Complex Care Support Workers to join a dedicated care package in Grimsby. You will provide person-centred care and support within the individual's home, following detailed care plans and clinical guidance.

We offer 12-hour shifts (days and nights) on a bank/0-hour contract, with full training and ongoing support from a specialist care team to help you develop skills in complex care scenarios.

Qualifications

• Experience in complex care, supported living, residential care or community/home care.
• Compassionate and person-centred approach to care.
• Reliable and punctual with good attendance.
• Confident in working as part of a team, including long shifts.
• Able to follow care plans and clinical instructions precisely.
• Confident communication with colleagues and healthcare professionals.
• Committed to delivering high-quality, dignified care.
• Demonstrates understanding of safeguarding and professional boundaries.
• Desirable experience with PEG care, tracheostomy, moving and handling, complex medication, enteral feeding.

Responsibilities

• Providing person-centred care and support within the individual's home.
• Following care plans, risk assessments and clinical guidance.
• Supporting personal care and daily living activities.
• Monitoring and reporting changes in health or wellbeing.
• Assisting with mobility and transfers where required.
• Working with the care team to ensure safe, consistent care.
• Maintaining accurate and timely care records.
• Communicating effectively with family, colleagues and healthcare professionals.
• Promoting dignity, choice, independence and wellbeing at all times.
• Adhering to safeguarding, health and safety and infection prevention procedures.
