Allscreens Nationwide Ltd is recruiting a Complex Care Support Worker to join a regional team across England and Wales. The role offers variety, independence and the chance to make a real difference when it matters most.

You will support complex care packages, aid hospital discharges, and mentor newer staff while working closely with Care Managers and clinicians to ensure timely, safe care in clients' homes.

Qualifications

• Full UK Driving Licence.
• Experience supporting people with complex care needs.
• Confident working independently and adapting to new environments.
• Excellent communication and organisational skills.
• Calm, compassionate and professional approach.
• Willingness to travel regionally.

Responsibilities

• Support the set-up, stabilisation and delivery of complex care packages.
• Support safe and timely hospital discharges.
• Deliver high-quality, person-centred care.
• Support clients with daily living activities and promote independence.
• Build routines that improve confidence and wellbeing.
• Provide reassurance and continuity for clients and their families.
• Mentor and support new Support Workers.
• Work alongside Care Managers and Clinical Professionals.
• Ensure medication, equipment and care supplies are in place.
• Identify concerns and escalate appropriately.

Complex Care Specialist Support Worker, you'll be part of a highly skilled team providing urgent support wherever it's needed across England and Wales. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance.

This is a role where your experience, compassion and confidence genuinely change lives.

Travel is an essential part of this role:

This is a regional role so you will need to be happy and able to travel across your designated region

This may include:

• Travelling to assignments across the country
• Working on both live-in and live-out care packages
• Staying away from home when required

To support you, we'll provide:

• Paid business mileage
• Accommodation for assignments outside reasonable travelling distance
• Accommodation for live-out packages where overnight stays are required
