Job summary

Active Care Group is seeking an experienced Complex Care Specialist Support Worker to provide urgent support across your region. You’ll help with hospital discharges, deliver person-centred care, and stabilise complex care packages, ensuring safety and continuity for clients and families.

This regional role requires a full UK driving licence, flexibility to travel, and the ability to work independently within a skilled clinical team.

Qualifications

• Full UK Driving Licence required.
• Experience supporting people with complex care needs.

• Flexibility to travel regionally.

Responsibilities

• Support set-up, stabilisation and delivery of complex care packages.
• Assist with safe hospital discharges and promote independence.
• Provide high-quality, person-centred care.
• Mentor and support new Support Workers.
• Work with Care Managers and Clinical Professionals to coordinate care.
• Ensure medications, equipment and supplies are in place and up-to-date.

As a Complex Care Specialist Support Worker , you’ll be part of a highly skilled team providing urgent support wherever it’s needed across your assigned region. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance. This is a role where your experience, compassion and confidence genuinely change lives.

You’ll support the set-up, stabilisation and delivery of complex care packages, including:

• Supporting safe and timely hospital discharges
• Delivering high-quality, person-centred care
• Supporting clients with daily living activities and promoting independence
• Building routines that improve confidence and wellbeing
• Providing reassurance and continuity for clients and their families
• Mentoring and supporting new Support Workers
• Working alongside Care Managers and Clinical Professionals
• Ensuring medication, equipment and care supplies are in place
• Identifying concerns and escalating appropriately

Travel is an essential part of this role: This is a regional role so you will need to be happy and able to travel across your designated region.
