Active Care Group is seeking an experienced Complex Care Specialist Support Worker to provide urgent support across your region. You’ll help with hospital discharges, deliver person-centred care, and stabilise complex care packages, ensuring safety and continuity for clients and families.
This regional role requires a full UK driving licence, flexibility to travel, and the ability to work independently within a skilled clinical team.
