Job summary

Tripod Partners are recruiting an experienced Adults Social Worker to join the Complex Care Team at Halton Borough Council in Halton. This role focuses on complex adult care, safeguarding, and developing person-centred plans.

The ideal candidate will manage varied caseloads, complete Care Act assessments, and collaborate with health and partner agencies to promote independence and wellbeing.

Qualifications

• A recognised Social Work qualification.
• Current Social Work England registration.
• Experience working within Adult Social Care.
• Experience managing complex cases.
• Strong assessment and care planning skills.
• Excellent communication and organisational skills.
• The ability to work independently and as part of a wider team.

Responsibilities

• Manage a varied and complex caseload.
• Complete Care Act assessments and reviews.
• Develop person-centred care and support plans.
• Assess and manage risk effectively.
• Work with service users, families and carers.
• Promote independence, choice and wellbeing.
• Work closely with health and other partner agencies.
• Undertake safeguarding work where required.
