Job summary

Tripod Partners is seeking an experienced Senior Adults Social Worker to join the Sutton Locality Team in London Borough of Sutton on a temporary contract. The role focuses on delivering high-quality assessments, care planning, and safeguarding within the Care Act framework.

You will manage complex caseloads, lead multi-disciplinary practice, and work closely with ICB, CHC, CMHT, housing and care providers to ensure person-centred outcomes for service users and carers.

Qualifications

• A recognised Social Work qualification and SW England registration.
• Significant Adult Social Care experience in a similar remit.
• Strong knowledge of the Care Act 2014 and safeguarding practices.
• Ability to lead complex assessments and multi-agency collaboration.
• Excellent report-writing and communication skills.

Responsibilities

• Deliver high-quality adult social work assessments and care plans.
• Manage a complex adult social care caseload and risk level.
• Complete Care Act assessments and reviews in a timely manner.
• Undertake carers' assessments and develop person-centred plans.
• Lead safeguarding enquiries under Section 42 when required.
• Manage complex transitions from Children's Services as needed.
• Chair safeguarding meetings and coordinate multi-agency panels.
• Prepare professional reports, care plans and panel applications.
• Provide guidance to less experienced colleagues and contribute to service audits.

Tripod Partners are currently recruiting for an experienced Senior Adults Social Worker to join the Sutton Locality Team at the London Borough of Sutton.

This is an excellent opportunity for an experienced Adult Social Worker who is confident managing complex cases and working within a multi-agency environment.

The Role

The successful candidate will deliver high-quality, strengths-based adult social work assessments, reviews and risk management interventions under the Care Act 2014 and associated legislation.

You will:

• Manage a complex Adult Social Care caseload
• Complete Care Act assessments and reviews
• Undertake carers' assessments
• Develop person-centred support plans
• Undertake complex safeguarding enquiries under Section 42
• Manage complex transitions from Children's Services
• Complete complex risk assessments
• Undertake Mental Capacity Act assessments
• Lead Best Interests decision-making processes
• Work within DoLS frameworks where required
• Develop and implement multi-agency safety plans
• Support crisis intervention and risk escalation
• Work closely with ICB, CHC, CMHT, housing and care providers
• Participate in multi-agency panels
• Support CHC assessments and funding applications
• Undertake face-to-face visits
• Chair and participate in safeguarding meetings
• Prepare professional reports, care plans and panel applications
• Support service users, families and advocates through complex transitions
• Participate in duty rotas and respond to urgent referrals
• Provide professional support and guidance to less experienced colleagues
• Contribute to service improvement and audit activity

About You

The ideal candidate will have:

• A recognised Social Work qualification
• Current Social Work England registration
• Significant Adult Social Care experience
• Strong knowledge of the Care Act 2014
• Experience managing complex adult cases
• Strong safeguarding experience
• Knowledge and practical experience of the Mental Capacity Act
• Experience working with multi-disciplinary and multi-agency partners
• Strong risk management skills
• Excellent assessment and care planning abilities
• The confidence to work autonomously
• Strong communication, negotiation and report-writing skills
