A leading complex care provider is seeking a Community Nurse Case Manager for the Greater Manchester area. This full-time position involves managing a caseload of clients, conducting assessments, and supporting a team of non-clinical staff. Ideal candidates will have RGN qualifications and expertise in complex care management, including tracheostomy and ventilator support.

The Nurse Case Manager will manage a caseload of clients with complex needs in the community. The role is varied and entails:
We are looking for an RGN Qualified Nurse with excellent clinical skills in complex care, including PEG feeds, ventilators, and tracheostomy management.
All sites are rated as Good with CQC, with some elements of Outstanding.
To apply for this role, you must be a car driver as the role is community-based.
This is a full-time community role, mainly Monday to Friday, 9am - 5pm, with some flexibility required, and includes travel across Greater Manchester.
The salary is negotiable depending on experience, up to £38,000, plus the opportunity to earn up to £6,000 per year in tax-free bonuses based on targets.
Other benefits include 25 days of annual leave plus bank holidays, an extra day for your birthday, pension, and support around CPD/Revalidation.
Tracheostomy experience is essential for this role.
