You will be a key contact for care teams, families and multidisciplinary professionals.

Qualifications

• Previous experience in care coordination or complex care.
• Experience coordinating staff rotas and ensuring safe service coverage.
• Strong understanding of person-centred care.
• Experience engaging with families and multidisciplinary professionals.

Responsabilités

• Coordinate day-to-day delivery of allocated complex-care packages.
• Create and maintain staff rotas and ensure appropriate staffing levels.
• Monitor service delivery and escalate risks promptly.
