You will assist with personal care, mobility, medication support per care plans, and help individuals engage with daily activities and community services, while maintaining dignity, confidentiality, and health and safety

Qualifikationen

• Experience in care or support roles preferred.
• Ability to work with individuals with diverse needs.

• Provide person-centered care and practical support to individuals who are elderly, disabled, or have health needs.
• Assist with personal care, daily living activities, and mobility.
• Support medication administration per care plans and policies.
• Promote independence, dignity, choice and wellbeing.

• Assist individuals with appointments, activities and community involvement.
• Maintain accurate care records and follow care plans.
• Safeguard individuals from abuse, neglect and harm.
• Collaborate with families, nurses, social workers and other healthcare professionals.
• Ensure confidentiality and follow health, safety and infection-control procedures.
