Job summary

Real Life Options is seeking a dedicated Support Worker to provide person-centred support in Drumchapel and surrounding areas. You will assist with personal care, medication support, daily tasks and community involvement, helping individuals to live their chosen life with independence.

• Must be able to work as part of a team and lone working; take initiative and problem-solving skills.

Responsibilities

• Support individuals with personal care, medication, cooking, and household tasks
• Enable involvement in the local community and promote independence
• Support with making and attending health care appointments
• Communicate with medical professionals and implement support plans and risk assessments
• Maintain clear and accurate records of support provided
• Communicate effectively with the team, family members
