Job summary

Harbor is currently looking to add a Case Manager to our mental health team in Toledo. This full-time position starts at $16.89/hr and requires a High School Diploma with 3 years of experience (Bachelor’s preferred).

You will document services, assess client needs, and coordinate ISP-driven care with families and providers. CPR/First Aid and CPI/NCI certifications are required within 90 days, with ongoing training and supervision.

Qualifications

• Minimum of a High School Diploma and 3 years of experience required.
• Bachelor’s degree in Counseling, Social Work, Psychology or closely related behavioral/mental health field preferred.
• Must be proficient and accurate in computer use, including Microsoft Word.
• Must successfully complete required training, before services are rendered, in basic competencies including an understanding of mental illness or substance use disorder treatment and recovery; crisis procedures; community behavioral health and other healthcare systems.

Responsibilities

• Responsible for accurate and timely documentation including activity/progress notes, ISP, DPS logs and incident report forms.
• Assesses client needs and formulates a strength-based ISP with client and family.
• Crisis prevention & intervention.
• Provides support, education and consultation to clients, families, and significant others.
• Monitors clients to identify and minimize effects of psychiatric symptoms.
• Coordinates evaluations and assessments by internal and external providers.
• Coordinates and monitors all services identified in the ISP.
