Responsibilities

• Provide job coaching to individuals with disabilities.
• Conduct evaluation for participant suitability for job placement.
• Document daily progress notes and job coaching plans.
• Maintains active communication with funding source and supervisor regarding client progress.
• Provides Job Coaching, JSST, and/or job development activities for opportunities for Ohioans with Disabilities.
• Evaluates work site for safety, supervision, and workload suitability.
• Analyzes job duties and environments, breaking them into trainable steps.
• Formulates interventions to help client maintain employment.
• Assist client with entitlements/benefits information to aid informed choices.
• Provides individual and/or group job coaching at the work site.
• Consults with supervisor on referrals to vocational or community programs.
• Delivers support and education to clients, families, and others.
• Helps participants solve barriers like transportation or childcare.
• Keeps current with trends in essential job competencies.
• May connect clients with community resources and vocational services.

Harbor is currently looking to add a Supported Employment Job Coach to our vocational team in the Toledo area. This position provides job coaching to individuals with disabilities at their job site to help them build the skills necessary to become independent at their job.

Position is contingent, working on an as-needed basis.

Education/Experience/Other Requirements:

• Two years teaching/training experience required. Bachelor’s degree with teaching/training experience preferred.
• Must be proficient and accurate in computer use, including Microsoft Word.
• Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
• CPR/First Aid Certification required within 90 days of employment.
• Must successfully complete a BCI/FBI background check(s) and six exclusionary database checks every 5 years, as required by OOD.

Essential Job Competencies/Primary Duties:

DOCUMENTATION

• Responsible for the quality and completeness of the electronic record.
• Responsible for accurate and timely documentation which includes, but is not limited to: daily progress notes, job coaching plans, Incident Report forms and any other forms necessary to document coaching services.
• Types documentation in the format appropriate and acceptable to Harbor.
• Maintains active communication with funding source and supervisor regarding client progress.

EVALUATION

• Provides Job Coaching, Job Seeking Skills Training (JSST), and/or job development activities for Opportunities for Ohioans with Disabilities
• Evaluates work site for appropriateness for participant placement (e.g., safety, supervision availability, enough work to keep participants busy, etc.).
• Analyzes job duties and environments at work sites; breaks down jobs into trainable steps for client.
• Formulates appropriate interventions to assist the client in making progress towards maintaining their employment.
• May assist the client in gaining information on the effects of employment on entitlements / financial assistance that the client may be receiving (i.e., SSI/SSDI, Medicaid, Medicare, Section 8, Food Stamps) to assist the client in making an informed choice.

ONGOING INTERVENTIONS

• Provides individual and/or group job coaching at work site.
• Consults with supervisor regarding appropriate referrals for client to vocational or community programs/services.
• Provides support, education (i.e., benefits counseling, risks and benefits of employment, and job seeking skills) and consultation to clients, families, and/or significant others.
• Provides support to participants to assist them in problem solving and managing barriers to client participation and/or employment (e.g., linking with childcare resources, health care resources, transportation, etc.).
• Keeps current with trends and developments related to essential job competencies, and demonstrates continued growth.

COLLABORATION

• May assist the client and family in increasing social support skills and networks.
• May coordinate evaluations and assessments by internal and/or external providers.
• Monitors all services identified in the referral.
• May provide assistance in gaining access to essential community resources and linking with appropriate vocational resources (OOD, ODJFS, BWC, etc.).
