Job summary

Harbor is seeking a Behavioral Health Case Manager to join the Toledo team. This full-time role delivers community-based case management for adults with mental health and behavioral health challenges, focusing on access to resources, daily functioning, independence, and community integration.

Responsibilities include coordinating care, maintaining documentation, delivering CPST/TBS services, and developing individualized treatment plans with clients and supports.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, or a closely related behavioral/mental health field from an accredited college or university.
• High School Diploma or GED and 3 years of experience in a behavioral health or related field may be accepted in lieu of Bachelor’s degree.
• Proficiency in computer use including MS Office 365, electronic health records, and virtual meeting platforms.
• Meets QMHS requirements and completes required training before services are rendered; includes crisis response, system understanding, and engagement techniques.
• CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

Responsibilities

• Serves as the primary clinician for assigned clients, coordinating care across providers and updating ISP regularly.
• Completes documentation such as progress notes, ISPs, needs assessments, and discharge summaries on time.
• Provides CPST and TBS services and may facilitate Day Treatment groups.
• Coordinates individualized treatment planning with client, family, and other providers.
• Develops and delivers evidence-based interventions and supports daily living and social skills.
