Job summary

Centred is seeking a dedicated Support Worker to provide high-quality care at home, helping individuals live safely, independently and with dignity. The role is 37.5 hours per week, on a rolling 4 days on / 4 days off rota with 12‑hour shifts.

Responsibilities

• Provide person-centred support to individuals in their daily activities.
• Promote independence, choice, and wellbeing.
• Maintain accurate and professional care records.
• Communicate effectively with service users, families, and external agencies.
• Work flexibly as part of a committed team.
• Work with support plans, risk assessments, and policies.
• Assist with medication administration.
