Job summary

Community Case Management Limited is seeking experienced support workers to join a small, compassionate team caring for a young woman with complex health needs in her home and community. The role combines personal care, rehabilitation, and practical support to help her stay independent and participate in meaningful activities.

You will work with therapists and colleagues, follow care plans and risk assessments, and maintain clear records.

Qualifications

• Experience supporting people with complex health conditions.
• Excellent communication and record keeping.
• Flexible to cover a range of shifts, sometimes at short notice.
• Physically able to assist with daily living.
• Enhanced DBS Disclosure.

Responsibilities

• Personal care and daily living: Providing respectful support with personal care and everyday activities.
• Rehabilitation: Following agreed rehabilitation programmes with therapists.
• Choice and independence: Supporting involvement in decisions about care.
• Social and community participation: Assisting in activities at home and in the community.
• Emotional wellbeing: Building a trusting professional relationship while maintaining boundaries.
• Safe and consistent support: Following care plans and reporting changes promptly.
• Communication and record keeping: Maintaining clear records and handovers.
• Teamwork and development: Participating in supervision and training.
• Delegated healthcare tasks: Carrying out within role after training and competence.
