Job summary

HR 4 Case Management is seeking a Female Support Worker to assist Lucy in her family home near Hereford. The role requires weekend shifts totaling 48 hours per week, with a mix of day and sleep-in hours and a driver with own vehicle to travel to the rural location.

You will support Lucy’s daily care, participate in therapy programmes, and document care notes. A caring, creative approach and team collaboration with family and therapists are essential.

Qualifications

• Nursing or clinical background is preferred.
• Must be an animal lover and comfortable with dogs.
• Full, clean UK driving licence and own vehicle are required.
• Excellent written and spoken communication skills are essential.

Responsibilities

• Support Lucy with daily personal care and clinical tasks.
• Implement and support Lucy's therapy programmes including hydrotherapy.
• Maintain daily care notes and clinical records accurately.
• Work as part of a team with family and therapists and accompany Lucy in the community.
