Job summary

Portsmouth City Council's Wellbeing Service is seeking a motivated public health practitioner to join the team delivering health improvement across PO1–PO6. You will support individuals to change behaviours, reduce health inequalities and promote wellbeing through mentored, community-based approaches.

You will assess needs, run group and 1:1 sessions, coordinate with GP practices, hospitals and community centres, and supervise staff where needed.

Qualifications

• Experience in a customer-facing role with clients (1-1s or groups).
• Ability to facilitate support with smoking cessation, healthy weight interventions, physical activity, alcohol reduction, emotional wellbeing and personalised support planning.
• Knowledge of Portsmouth's demographic and the wider determinants of health.
• Good analytical, communication and organisational skills.
• Right to Work in the UK.

Responsibilities

• Lead initial assessments and identify support needs.
• Foster community involvement and connect people with appropriate support.
• Lead community-based clinics and supervise staff in small to medium-sized teams.
• Be the point of contact for advice and information within your area of expertise.
