Job summary

Portsmouth City Council is seeking a Wellbeing Practitioner to deliver public health services across Portsmouth’s PO1–PO6 venues. You will support behaviour change to reduce health inequalities, addressing smoking, obesity, physical activity and mental wellbeing in diverse communities.

You will assess client needs, develop personalised support plans, and may supervise junior staff. The role involves evening shifts and travel around the city to multiple venues and GP practices.

Qualifications

• Experience in customer-facing roles delivering behaviour change interventions.
• Ability to facilitate support with smoking cessation, healthy weight, physical activity, and emotional wellbeing.

Responsibilities

• Conduct initial assessments and identify support needs through personalised planning.
• Deliver public health interventions across venues, including GP practices, hospital settings and community centres, plus telephone/virtual support.
• Mentor and support less experienced staff and supervise small to medium teams.
• Travel across the City to work in PO1-PO6 areas and transport resources to venues.
