Job summary

Portsmouth City Council’s Wellbeing Service helps people improve health and reduce inequalities across the PO1-PO6 area, in GP practices, hospitals, community centres and via phone/online support. The role involves assessing needs, promoting wellbeing, and linking individuals to appropriate services.

You may work evenings and will travel to varied venues, supervise staff, mentor new team members, and use data/IT tools to manage caseloads.

Qualifications

• Experience in customer facing role with clients (1-1s or group).
• Ability to facilitate support with smoking cessation, healthy weight, physical activity, alcohol reduction, emotional wellbeing.
• Knowledge of Portsmouth's demographics and wider determinants of health.
• Knowledge of recognised behaviour change models and techniques.

Responsibilities

• Engage with clients across venues, identify support needs through initial assessment.
• Foster community involvement and connect people with appropriate support.
• Potential supervision of staff and mentoring of new staff development.
• Travel across the City to deliver services in PO1-PO6 area.
