Join the Moor to Sea Community Nursing Team

The Moor to Sea Community Nurse team covers Ashburton, Buckfastleigh, Dartmouth, Totnes, and surrounding areas. We are a busy, friendly, and supportive team committed to staff development through education and training.

We focus on helping staff realize their potential in topics of interest and encourage career pathways in Community Nursing. We are aware of the pressures faced by nursing staff and how these can impact overall wellbeing. We are open to listening to reasonable adjustments to improve the working experience.

Our team prides itself on delivering high standards of care and building relationships within our communities. We work towards a more integrated approach to care delivery, collaborating closely with colleagues in Intermediate Care, Social Care, and the Voluntary Sector to achieve this.

Responsibilities
  • Providing direct contact with patients, including personal care, clinical interventions, and supporting health and wellbeing, tailored to individual needs as outlined in care plans.
  • Ensuring compliance with Trust and Department policies according to health and safety guidelines.
  • Reporting any actual or potential risks to patients, staff, or the Trust to the nurse in charge.
  • Assessing risks comprehensively to aid in their management or prevention.
