Job summary

Central London Community Health Trust is seeking a Band 3 Community Support Worker to join the Brent Urgent Community Response Team. The role involves delivering safe, compassionate care in patients' own homes within a busy community setting, working with nurses, therapists and other professionals.

You will provide holistic support, help patients maintain independence, and carry out delegated clinical activities under supervision.

Qualifications

• Full UK driving licence and access to a car are essential.
• Ability to communicate effectively with patients, families and colleagues.

Responsibilities

• Provide safe, compassionate and personalised care to patients in their own homes, working alongside registered nurses, therapists and other members of the multidisciplinary team.
• Support patients with nursing and therapy needs, helping them to maintain their independence and remain safely at home wherever possible.
• Carry out delegated clinical activities and interventions under the direction of a registered healthcare professional and within your agreed level of competence.
• Provide holistic, person-centred support, recognising changes in a patient's needs or condition and reporting concerns appropriately.
• Support the team with relevant clinical and administrative duties to ensure patients receive timely and coordinated care.
• Communicate effectively and sensitively with patients, families, carers and healthcare professionals.
• Maintain accurate patient records and confidently use the relevant IT systems required for the role.
• Work collaboratively as part of the wider community team to deliver a responsive and effective service.

Job description

Are you a caring, compassionate and motivated individual who is passionate about supporting people to remain safe and independent in their own homes?, This is a full-time role, working 37.5 hours per week, supporting the delivery of high-quality care to patients within their own homes and community settings. Working as part of a supportive multidisciplinary team, you will help provide timely care and support to patients who may be experiencing a change in their health or circumstances. You will play an important role in helping patients maintain their independence, supporting their recovery and preventing avoidable hospital admissions wherever possible., As a Band 3 Community Support Worker within the Brent Urgent Community Response Team, you will:

• Provide safe, compassionate and personalised care to patients in their own homes, working alongside registered nurses, therapists and other members of the multidisciplinary team.
• Support patients with a range of nursing and therapy needs, helping them to maintain their independence and remain safely at home wherever possible.
