Job summary

HCRG Care Group seeks a Community Nurse (Band 5) to deliver patient-centred clinical care in community settings from Beacon House. You will assess, plan, implement and evaluate care, working within the nursing process and alongside a multi-disciplinary team.

You will supervise junior staff, manage a caseload, and maintain high standards of documentation and professional practice. Requires NMC registration and a nursing degree, with a commitment to development.

Qualifications

• Registered nurse with NMC and current practising status.
• Must have nursing degree qualification and healthcare experience.
• Experience in multidisciplinary team care in the community.
• Able to manage fluctuating workload and complex patient needs.
• Competent with IT systems including SystmOne, Word and Excel.
• Capable of safe medication administration per policy.
• Involvement in mentoring and developing junior staff.
• Evidence of engagement in clinical audit and service improvement.

Job description

The Integrated Neighbourhood team is a community-based service which supports and works with people, either in their own home or in Residentlal or Nursing Homes. The role of the community nurse is to work with the multi professional team within community serviceproviding clinical support in assessing, planning, organising, and delivering clinical care in line with serviceand patient needs, as agreed with supervising clinicians. The community nurse works independently, working within the sphere of nursing care and within all aspectsof the nursing process. The post holder will act professionally at all times and use their knowledge andexperience to make evidence-based decisions to solve challenges and problems. They recognse and workwithin the limits of ther competencies and are responsible and accountable for their actions. The post holder will have responsibility for supervising less experienced or qualified staff and studentsduring their placements. They will be expected to actively contribute to quality assurance processes andservice development.,

• Understands and acts in line with NMC professional standards for practice.
• Working as a member of a skill mixed team, responsible for interacting constructively with teammembers and organising, prioritising, and planning their work.
• Uses a range of assessment tools pertinent to the patients' needs, to inform the assessment and assess risk for both patients and staff.
• Demonstrates an ability to prioritise and manage their own workload and recognise where elements of care can safely be delegated to other colleagues, carers, and family members.
• Facilitates the provision of care for people, including those with complex needs.
• To keep the patient at the centre of their care, ensuring patient and carer participation in decisionmaking.
• Provide sensitive, compassionate, safe, and effective care to support people in a range of care settings, applying the principles and processes for making reasonable adjustments.
• Implenment all elements of the nursing process including the on-going assessment, planning, management, and evaluation of care.
• Manage own caseload within scope of practiceUndertake clinical tasks including observations, catheterisation, venepuncture and ECGs. This is notan exhaustive list; additional competency-based skills relevant to clinical area can be attained.
  Essential:
• Nursing Registration (NMC).
• Nursing degree qualification.
• Previous experience of working in a healthcare setting as part of the multi-disciplinary team.
• Able to deal with non-routine and unpredictable nature of the workload and individual patientcontact.
• Proficient in using a variety of IT systems for example SystmOne, word, excel.
• Able to competently and safely administer medication according to organisation policy.Able to support, supervise, assess, and act as a role model.
• Passionate about delivering safe patient focused care.
• Ability to work on own initiative and as a team member.
• Good time management skills.
• Valid UK driving license with access to a vehicle for work purposes.
• Shows commitment to ongoing personal and professional development.
