Job overview
The Tower Hamlets Stroke Team provides seamless care from admission to the hyper acute and acute stroke units at the Royal London Hospital through to discharge supported by the community team. The post holder would work within a strong supportive MDT with a focus on encouraging a self-management approach, and overseeing programmes carried out by rehab support workers.
The post holder will require a background in stroke care and must have experience of multi-disciplinary team-working. Experience of working in the community and knowledge of complex discharge planning would also be beneficial.
The wider Barts Health Speech and Language Therapy service covers a large part of the culturally and economically diverse population of East London and covers 4 acute hospital sites and two community areas. You will be joining a large SLT department based across these sites with specialist expertise across the patient journey.
Supervision and support for this role specifically will be provided by the Highly Specialist SLTs (Community Stroke Team & Early Supported Discharge) as well as the Clinical Lead SLT based at RLH. There is regular clinical supervision, peer support and continuing professional development as well as annual appraisal.

Main duties of the job
This band 6 SLT post (0.5 WTE) is based within the Tower Hamlets Community Stroke Team. The post holder will work closely with colleagues on the Acute Stroke Unit at the Royal London Hospital, with the opportunity to provide in-reach, as part of the Early Supported Discharge model.
The post holder will be part of a multi-disciplinary team providing specialist assessment and treatment to adults with acquired neurological communication and swallowing difficulties, as a result of stroke. The Team has a focus on fostering a self-management approach with service users and their families.
