Berkshire Healthcare NHS Foundation Trust in Reading is looking for a Community Staff Nurse to join the Reading Community Nursing team. You will assess, treat, and support patients in their own homes, delivering high-quality, evidence-based care across a wide range of conditions.

You'll work full-time in a service operating 7 days a week, with induction and ongoing development, opportunities for progression, and access to a comprehensive benefits package including flexible working and staff

Formación

• People skills to work effectively with colleagues and patients – with excellent listening and communication skills.
• A driving license and access to a car.
• Calm approach and ability to prioritise and work within a busy team.

Responsabilidades

• To holistically assess the needs of patients and carers, and to plan, implement and evaluate programmes of nursing care.
• To undertake first visits and take immediate action, reporting back to senior staff for further discussion.
• To provide care according to best practice and support development of evidence-based care.
