Job summary

Leeds Community Healthcare NHS Trust is seeking a passionate Community Staff Nurse to join our out-of-hours team in Leeds. You will provide compassionate patient care in the community with a focus on assessment, planning and timely delivery of services.

We offer development plans, clinical supervision and access to a peer support network, with a rota running 17:00–22:00 across seven days a week. You must have a full driving licence and access to a vehicle; sponsorship is not offered.

Qualifications

  • Must be a registered staff nurse with strong assessment, planning and evaluation skills.
  • Ability to work with patients, families and multidisciplinary teams.
  • Provide safe and effective clinical care in a fast-paced community setting.

Responsibilities

You will be a registered staff nurse and have excellent assessment, planning and evaluation skills to ensure that patient care is timely and appropriate.

You will have the opportunity to work closely with patients, families, and a multidisciplinary team to ensure the best possible outcomes.

Your responsibilities will include monitoring patients, administering medications, coordinating care plans, and providing support to families/carers during different crisis situations – i.e, blocked catheters and palliative care support.

This is a fast paced role requiring safe and effective clinical decision making, so you must be able to think on your feet, problem solve issues that arise and juggle conflicting priorities. You must be able to motivate others in challenging times and utilise the resources available to you effectively.

The hours of work are 17:00pm - 22:00pm over 7 days a week (including bank holidays).

It would be expected that you are available to work on a rota over 7 evenings per week, 365 days per year.

The contract is for a min of 10 hours (2 evenings) up to a max of 27.5 hours (5, 6 evenings). Hours can be discussed prior to application or at interview as required.

You must have a full driving licence and access to a vehicle suitable for work purposes as driving citywide is an expectation of the role.
