Job summary

The Royal Wolverhampton NHS Trust is seeking a motivated Community Staff Nurse with a passion for wound care to join our specialist wound care service within community settings. You will assess and manage complex wounds, deliver evidence‑based care and work autonomously in a multidisciplinary team to optimise healing outcomes.

You will also perform ABPI studies, compression therapy, catheter and nephrostomy care, and maintain thorough clinical documentation in line with NMC standards.

Job description

We are seeking a motivated, confident and clinically skilled CommunityStaff Nurse to join our specialist wound care service. This role is ideal for a nurse with a passion for wound management, who thrives both independently and within a collaborative multidisciplinary environment.

You will work within our community clinic settings, reviewing ambulant patients and delivering high‑quality, evidence‑based care to support safe and positive wound‑healing outcomes. The role involves managing a varied caseload of complex wounds, utilising tissue viability guidance and vascular oversight to ensure safe, effective and innovative care planning.

Main duties of the job

You will work within our clinic settings, reviewing patients to support positive wound‑healing outcomes. Your day‑to‑day responsibilities will include comprehensive wound assessments, wound reviews, treatment planning, ABPI studies, compression bandage assessment and application, and delivering holistic patient assessments.

You will be expected to deliver a consistently high standard of care, demonstrating a proactive approach to clinical practice and utilising innovative, evidence based - care planning to support optimal patient outcomes.

The role also requires competence in catheter care, nephrostomymanagement, and close collaborative working with wider services. You will work in partnership with patients to ensure a safe, effective andpositive patient experience throughout their care journey.

Detailed job description and main responsibilities

The Community Staff Nurse will provide high‑quality, evidence‑based nursing care to patients requiring complex wound management within clinic settings. The post‑holder will work autonomously and as part of a multidisciplinary team to ensure safe, effective and positive wound‑healing outcomes.

Clinic Duties.

Deliver comprehensive wound assessments, reviews and treatment planning for patients with complex or chronic wounds.

Utilise specialist knowledge in tissue viability, vascular assessment, and lymphoedema management to inform safe and effective care pathways.

Apply advanced wound care techniques, including compression therapy, negative pressure wound therapy (if applicable), and specialist dressings.

Conduct ABPI studies and interpret results to guide compression therapy and vascular referral decisions.

• Monitor wound progression, elevate concerns appropriately, and ensure timely intervention to optimise healing outcomes.

Provide holistic nursing assessments, considering physical, psychological and social factors impacting wound healing.

Deliver safe and competent catheter care, including urethral and suprapubic catheter management.

Provide nephrostomy care, ensuring correct maintenance, monitoring and escalation of complications.

Administer medications in line with local policy, ensuring safe storage, documentation and patient education.

Maintain accurate, timely and professional clinical records in accordance with NMC standards and organisational policy.

Work closely with Tissue Viability Nurses, Vascular Teams, Lymphoedema Services, Podiatry, District Nursing and other community‑based services.

Participate in multidisciplinary discussions to support coordinated care planning and seamless patient pathways.

Engage patients and carers in shared decision‑making, providing education and support to promote self‑management where appropriate.

Contribute to service development, quality improvement initiatives and the implementation of innovative care approaches.

Provide care within community clinic environments to ambulant patients requiring specialist wound care support.

Ensure a positive patient experience through compassionate communication, timely intervention and professional conduct.

Support safe caseload management and contribute to the smooth running of clinic operations.
