Community Staff Nurse - Dorset HealthCare

Dorset HealthCare is seeking a Community Staff Nurse to join our innovative Integrated Care Team in Weymouth and Portland, within the District and Community Nurse team.

Main Responsibilities:

1. Provide high-quality, person-centred care to patients in their homes, ensuring safety, privacy, and dignity.
2. Undertake patient assessments, develop care programmes, and work as part of a multidisciplinary team.
3. Manage acute and chronic nursing needs, build therapeutic relationships, and collaborate with community partners.
4. Monitor patient conditions, record observations, and help develop wellbeing care plans.
5. Support patients in maintaining independence, prevent unnecessary hospital admissions, and deliver end-of-life care.

Qualifications & Benefits:

• Flexible shift patterns between 08:00-20:00, with weekend enhanced pay.
• Structured induction, flexible working, and pastoral support.
• Opportunities for learning and development, including links with local universities and preceptorship for newly qualified nurses.
• Employment requires an Enhanced DBS check, which the Trust covers. Applicants can present a valid DBS certificate if subscribed to the update service.
