The role has evolved to focus on caring for complex patients in their home environments, requiring skills in complex wound management, palliative and end-of-life care, medicines management, IV drug administration, and holistic assessment and care planning.

Training will be provided within the Trust for the successful candidate.

If you seek a role that offers autonomy, challenges, and career development opportunities, consider joining our community team. We work closely with multidisciplinary teams including local hospices, complex care nurses, health and social care coordinators, specialist nurses, and therapists. Our team is supported by the Intermediate Care Service and utilizes community hospitals for patient rehabilitation.

Note: Applications from candidates requiring current Skilled Worker sponsorship are unfortunately ineligible for this position.

Key Responsibilities

  • Delivering high-quality, person-centred care that respects safety, privacy, and dignity.
  • Assessing patients' needs and developing care plans in collaboration with the multidisciplinary team.
  • Offering advice to patients, carers, and families regarding care.
  • Planning, implementing, and reviewing health improvement programs across various settings.
  • Supervising, mentoring, and providing clinical support to students and junior staff.

Employment requires an Enhanced DBS check, which the Trust will cover. Applicants subscribing to the DBS update service can present a valid certificate.
