Community Staff Nurse

Are you a registered nurse looking for a role inDistrict Nursing. We are looking for an experienced and motivated individualwho will be responsible for carrying out holistic and autonomous evidenced-basednursing care to patients under the care of the Warrington District Nursingservice. You should be able to demonstrate ongoing professional development.

The post holder will provide a high standard ofcare, utilising clinical skills in the assessment, planning and implementationof pro-active care to a community population with nursing needs referred intothe district nursing service. The post holder will work in close collaborationwith other professionals/stakeholders within the Warrington footprint.

Main duties of the job

To promote the health and wellbeing within thefamily unit and community as a whole.

Perform comprehensive assessment of patient nursingneeds as the named nurse.

Plan, assessand evaluate programmes of care according to changing health needs under thedirection of the District Nurse team leader.

Tosupport the team leader in the co-ordination of complex and continuing carepackages.

Maintainaccurate records in accordance with NMC.

Attenda multi-disciplinary case conference as the named nurse if appropriate.

To communicate and collaborate with other members ofthe district nursing team , voluntary and statutory organisations to supportindividual patient care.

To establish and maintain effective communicationwith patients, relatives and carers in complex and potentially challengingsituations.

To competently perform clinical skills related tothe post.

To be professionally responsible for a delegatedcaseload under the supervision of the District Nurse.

To assess health needs at individual, practice andcommunity level.

To undertake chronic disease management reviewsusing appropriate template.

Job responsibilities

Thepost holder actively supports the District Nursing team by undertakingresponsibility for a delegated caseload.

Undertakesthe assessment, planning and evaluation of patients care needs.

Liaiseswith the District Nurse to ensure informationsharing in relation to patient care / changes to care plan and thecontinuity of care and reviewing of complex care packages.

Mentoring,Teaching and Assessing.

Liaiseswith all health care professionals and other agencies to ensure effectiveservice delivery.
