If so, then why not join us as a Community Staff Nurse, helping us deliver outstanding patient care in the Leeds community? In this role, you will work in one of our 13 Neighbourhood Teams. The service operates from 7:00am - 10:00pm, covering a 7-day roster period which includes weekends and Bank Holidays.
The Neighbourhood Team provides care for patients requiring intervention and support in their own homes. Patient needs are varied and can range from insulin injections & medication prompts to end-of-life care. We are part of an integrated team which includes therapy and social care.
The role is autonomous and covers a large geographical area, so having a car and a full licence is essential. You will be supported with a fully tailored induction programme and will have opportunities to develop during your employment with Leeds Community Healthcare NHS Trust.
This vacancy is open to Newly Qualified Nurses on the Community Pathway only.
As a Community Staff Nurse, you will visit patients in their own homes undertaking a variety of clinical interventions. You must have excellent communication, organisational, and IT skills. You will need to assess and respond to the ever-changing needs of the patient, working within your own limits and seeking support from the wider clinical team where appropriate.
