We offer development and training opportunities for our staff. We are currently seeking highly motivated, passionate Band 5 Community Nurses who are keen to develop into one of the following roles:

• Band 6 District Nurse by undertaking the District Nurse Qualification.
• Chronic Condition Case Manager by undertaking disease-specific training to provide evidence-based care to individuals with complex needs.
• Nurse Practitioners by undertaking a modular approach to attain the required clinical skills.

Main duties of the job

The successful applicants will need excellent communication skills and the ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across Anglesey and Gwynedd, working with partner organizations to deliver high-quality, evidence-based care aimed at maintaining patients in their homes and communities, reducing hospital admissions, empowering self-management, and providing seamless, patient-centered care.

You must be able to travel between patients' homes within towns and rural areas in a timely manner.

Fluency in Welsh is desirable; applications from Welsh and/or English speakers are equally welcome.
