The successful candidate will possess excellent communication skills and be able to deliver clinically effective care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board provides District Nursing Services across Conwy and Denbighshire, working with partner organizations to deliver high-quality, evidence-based care aimed at maintaining patients in their homes and communities, reducing hospital admissions, empowering self-management, and providing seamless, patient-centered care.
You must be able to travel between patients’ homes within towns and rural areas promptly.
This post operates on a rotational 24-hour schedule.
Welsh language skills are desirable; applications from Welsh and English speakers are equally encouraged.
