Job summary

Leicestershire Partnership NHS Trust invites Registered Adult Nurses to join our Community Nursing Teams, delivering compassionate care to patients in their own homes across Leicester, Leicestershire and Rutland. Posts are available for newly qualified nurses and students qualifying in January 2027.

This role offers flexible working, supportive hubs, autonomy in your practice, clinical and managerial supervision, and preceptorship for newly qualified nurses.

Responsibilities

• Providing clinical care including wound and leg ulcer management, palliative care, symptom control, and end-of-life care.
• Administering medications including IVs and supporting patients at home.
• Collaborating with the multidisciplinary team to promote independence and avoid hospital admission.
• Promoting self-care and empowering patients to remain at home.

The service operates 7 days a week, 08:00 to 22:00. Vacancies are available across Leicester, Leicestershire, and Rutland, with location preferences discussed at interview.

Job Description Summary (Main duties and responsibilities)

Main duties of the job

• Providing clinical care to include management of wounds and leg ulcers, care of patients with a palliative diagnosis, symptom control and care in the last days of life, bowel, continence and catheter care and administration of medication including IVs.
• Collaborating with the wider multidisciplinary team.
• Supporting patients to remain at home and avoid hospital admission.
• Promoting self-care and independence.

It is an essential requirement that you have a car for work use and have a UK driving licence.
