Job summary

The Hounslow Health Visiting Service is seeking a Band 5 Community Staff Nurse to support the delivery of the Healthy Child Programme.

The successful applicant will work within a skill mix team to support and assist in the development, implementation and evaluation of activities aimed at promoting the health of the local population, as well as offering support to identified families as delegated by a Health Visitor.

• Delegated Home visits

• Accident prevention and follow after A/E attendances
• To support Duty Health Visitor role
• To hold a small caseload of families to deliver delegated packages of care e.g. LAC children, children with SEND needs, premature infants , children with long-term healthcare conditions, listening visits. To report back to named HV and seek regular caseload supervision with the named HV.
