Job summary

East of England Community Health and Care NHS Trust is seeking a Registered Staff Nurse to join our community nursing team in North Norfolk. The 30-hour (4 day per week) role covers NN3 Team areas including Aylsham, Reepham, Drayton and Coltishall, within the Primary Care Network framework, linking GP practices, social services and care providers.

You will work under Band 6 and Clinical Lead supervision, delivering high-quality, holistic nursing care in various community settings.

Job description

We are looking for a Registered Staff Nurse to join our dedicated and highly professional team of community nurses in North Norfolk. We have a 30 hour (4 day per week) position within the friendly and welcoming NN3 Team (Aylsham, Reepham, Drayton and Coltishall areas)., The service is key to the on-going development of the locality within the new Primary Care Network, and will provide the link between our operational services and the GP practices. Ensuring effective communication for those in the care of NCH&C between the G.P practice, Social Services and other service providers involved their care to ensure best outcomes. You will demonstrate a proactive philosophy to care, committed to a "whole patient" approach, ensuring holistic care of the patient and involvement of all parties. You will be someone who believes strong and effective communication is essential and who strives to ensure quality patient care is their highest priority. To assist you in doing this you will be clinically supported by our Community Matrons, Clinical Leads and by a management team committed to patient centered quality care. You will receive support and development through in service training, regular appraisals, a personal development plan and clinical supervision. The Community Staff Nurse role is a role under the supervision of Band 6 and Clinical Lead colleagues as part of the larger integrated team, providing high quality patient centred nursing care to patients in a variety of community settings. Nursing care delivered will include catheter care, support for patients with palliative conditions including syringe driver management, management of pressure ulcers and legs ulcers as well as other chronic and surgical wounds, insulin and diabetes management and provision of holistic assessments. Candidates should have an ability to motivate and educate patients with the goal of promoting self management of their health care needs.
