Job summary

Sunderland GP Alliance is seeking a Social Prescribing Link Worker to support people in Sunderland to manage their health and wellbeing. You will work with GP practices, PCNs and communities to empower individuals to access services and build resilience.

Qualifications

• Ability to identify risk to self and others and report safeguarding incidents.
• Experience of working in a community setting and with GP teams.
• Proficient with IT systems to create simple personalised plans.

Responsibilities

• Take referrals and provide personalised social prescribing support within the PCN MDT.
• Develop and co-produce simple personalised care plans and connect to community groups.
• Educate non-clinical and clinical staff on available community services and access methods.
• Support referrals and manage caseload with sensitivity and confidentiality.
