Job summary

The Rehab Group in Ireland is seeking a Relief Programme Facilitator on a fixed-term contract (6 months) with variable hours at the Newport, Co Tipperary location. The role involves person-centred planning, developing programmes for independent living, personal development and activities, requiring QQI Level 5 in Social Care or equivalent, and experience with adults with intellectual disabilities.

Qualifications

• Minimum QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Experience supporting adults with an intellectual disability and people with mental health issues.
• Programme facilitation skills and client handling experience are advantageous.
• Experience of lone working and community support per New Directions.

Responsibilities

• Act as key worker to facilitate Person-Centred Planning and record keeping.
• Develop and deliver person-centred programmes to meet service user needs.
• Support individuals and groups to access community activities and maintain records.
• Work independently as a lone worker and liaise with the team to ensure quality service.
• Comply with Rehab Group rules and promote safe, respectful service delivery.
• Promote independence and community integration in collaboration with local stakeholders.
• Ensure health and safety and safe transport, using required safety equipment.
