Job summary

The Forward Trust is seeking a RECONNECT Practitioner to support people leaving prison and help them engage with health, wellbeing and recovery services in the community across Hertfordshire. You will work from HMP The Mount and surrounding area, delivering person-centred support from 12 weeks pre-release to up to six months post-release.

You will assess needs, develop SMART care plans, coordinate with healthcare providers, and help clients establish strong community connections.

Qualifications

• Experience completing comprehensive assessments and developing SMART care plans.
• Ability to manage a busy caseload with pre- and post-release support.
• Ability to coordinate with healthcare providers and partner organisations.
• Proficient in motivational interviewing and person-centred approaches.

Responsibilities

• Managing referrals, completing needs assessments, and providing feedback to referrers.
• Developing and implementing personalised care plans and risk assessments.
• Managing a varied caseload, providing guidance and ongoing support pre- and post-release.
• Supporting access to GP, mental health services and substance misuse treatment.
• Delivering one-to-one interventions using motivational interviewing.
