Job summary

City Health Care Partnership CIC in Hull and the East Riding is recruiting a Bank Generic Therapy Associate Practitioner to deliver community rehabilitation within a multidisciplinary team, under the supervision of a therapist, and to support patients to regain independence.

You will manage a defined caseload, carry out assessments and care planning with registered clinicians, and collaborate with patients, carers and partner organisations to ensure timely, high-quality care.

• Ability to work independently while knowing when to seek support from registered professionals.
• Commitment to teamwork and safe practice.

Responsibilities

• Deliver rehabilitation and wellbeing interventions to support independence.
• Support assessments, care planning and reviews with guidance from registered clinicians.
• Monitor patient progress and escalate concerns to ensure safe care.
• Collaborate with patients, carers and partner agencies to coordinate support.
• Supervise and support less experienced colleagues and support workers.
