Job Title: Band 5 Community Psychiatric Nurse – Dementia
Location: Shrewsbury
Contract: Monday to Friday, 9am–5pm (Full-time)
Type: Temporary NHS Agency Contract

Service Care Solutions are currently recruiting for a Band 5 Community Psychiatric Nurse to join a Dementia team in Shrewsbury. This is a fantastic opportunity to work within a supportive and well-established NHS community team.

Key Responsibilities:

• Manage a defined caseload under the supervision of a Band 6 Nurse, including specialist assessments, care planning, and evaluation of treatment plans in line with Care Programme Approach (CPA) and Single Assessment Process (SAP).
  
  
• Contribute to the setting and monitoring of quality standards, in line with clinical guidance and Trust policies.
  
  
• Administer medication in accordance with NMC guidelines and Trust procedures.
  
  
• Assess individual care needs and deliver tailored interventions based on agreed care plans and pathway guidance, including both pharmacological and psychologically informed approaches.
  
  
• Complete and contribute to comprehensive risk assessments and risk management plans, promoting positive risk-taking to enhance quality of life and independence.
  
  
• Maintain accurate documentation and provide verbal and written reports on patient progress and conditions.
  
  
• Use agreed outcome measures to assess and review the effectiveness of interventions as part of the care planning process.
  
  
• Support training and education of students, colleagues, carers, and service users through both formal and informal means.
  
  

Requirements:

• Registered Mental Health Nurse (RMN) qualification
  
  
• Experience working with individuals with dementia in a community setting
  
  
• Valid NMC registration
  
  
• Enhanced DBS – Adults
