Job summary

Sirona care & health CIC in Bristol, North Somerset or South Gloucestershire, is seeking a Band 5 Community Physiotherapist to support hospital discharge and home-based rehabilitation. 37.5 hours per week, including weekends on a rota, with full NHS benefits and a competitive Agenda for Change salary.

You will assess, plan and deliver personalised rehabilitation, work within an MDT, and contribute to service development while maintaining high standards of care in patients' homes.

Qualifications

• Bsc or equivalent in Physiotherapy and HCPC registered.
• Evidence of on-going CPD/Reflective Practice.
• Membership of Chartered Society of Physiotherapy.

Responsibilities

• Autonomous practitioner responsible for safe assessment and management of own caseload.
• Plan and prioritise workload; supervise assistants and students.
• Support service users in goal setting and deliver personalised treatment plans.
• Maintain accurate patient records and work with the MDT to achieve goals.
• Order equipment and facilitate referrals; report issues as needed.
• Identify risks and participate in service development and training.

Job overview

Our Therapists support hospital discharge, admission avoidance and long-term self-management, enabling patients to live as independently as possible in their own homes.

We provide rehabilitation in the patient’s place of residence, helping to maximise functional potential through tailored interventions, equipment provision and liaison with other services where required.

The role includes assessing functional ability and support needs, establishing meaningful rehabilitation goals, creating plans for support staff and identifying longer-term care requirements where appropriate.

We are seeking a motivated Physiotherapist to join our team and promote choice for our service users and their carers. You will be working alongside OT's, case managers and support workers and will develop overlapping skills with these professions.

In return, we'll give you the opportunity to develop skills and enhance your multidisciplinary working; we will provide you with regular supervision as well as the opportunity to develop your leadership skills.

We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs. Working arrangements include weekends and bank holidays on a rota basis.

Main duties of the job

• Be an autonomous practitioner responsible for safe and comprehensive assessment and management of own caseload.
• Work within your scope of practice.
• Plan and prioritise workload independently and supervise assistants, support workers and students as required.
• Support and guide service users in appropriate goal setting.
• Use clinical reasoning and treatment skills to deliver personalised treatment plans based on evidence based practice.
• Maintain accurate patient records in line with guidelines.
• Work as part of a wider MDT to achieve patient goals.
• Communicate effectively with the wider MDT and case-manage services users as appropriate.
• Delegate and supervise appropriate tasks and physiotherapy treatment plans to Rehabilitation Workers/ Physiotherapy Assistants/Technicians.
• Be able to order equipment and ensure its safe and appropriate use, reporting any issues appropriately.
• Facilitate referrals to other services based on your assessment to ensure holistic patient care.
• Identify risks and act accordingly to minimise risk to self, service users and colleagues.
• Actively participate in training; assisting in the delivery of training within the service and to other health professional as appropriate.
• Actively participate in service development projects as required.

Skills, Knowledge & Experience

• Good verbal and written communication skills with the ability to adapt communication for individuals with communication difficulties.
• Able to work autonomously and seek appropriate advice/guidance when required.
• Competent assessment and treatment skills including exercise prescription, gait analysis, falls assessment.
• Effective time management and caseload management.
• Experience of working in a community setting (either student or post graduate).
• Experience of the application of health, safety and risk management policies
