Job summary

Sirona care & health is recruiting a Physiotherapist to join our community team serving Bristol, North Somerset and South Gloucestershire. You’ll assess, treat and support patients at home, coordinating with OTs, case managers and support workers to maximise independence.

We offer Agenda for Change pay and full NHS benefits, including generous annual leave and access to the NHS Pension Scheme. This is a community role with flexible hours, including weekends on rota, to meet patient needs.

Qualifications

  • Autonomous practitioner capable of safe, comprehensive caseload management.
  • Ability to plan workload independently and supervise others.
  • Guide service users in appropriate goal setting and care planning.
  • Apply evidence-based practice in treatment plans.
  • Maintain accurate patient records and comply with guidelines.
  • Collaborate within a multidisciplinary team to achieve patient goals.
  • Delegate tasks to Rehabilitation Workers/ Physiotherapy Assistants/Technicians.
  • Order and ensure safe use of equipment; report issues promptly.
  • Facilitate referrals based on assessment findings.
  • Identify and manage risks to patients and staff.
  • Participate in training and service development projects.

  • Collaborate with the wider MDT to achieve patient goals.
  • Delegate treatments to Rehabilitation Workers/ PT Assistants/ Technicians.
  • Order equipment and ensure its safe use; report issues.
  • Refer to other services as appropriate after assessment.
  • Identify risks and minimise harm to self, patients and colleagues.
  • Participate in training and service development projects.

Job description

Our Therapists support hospital discharge, admission avoidance and long-term self-management, enabling patients to live as independently as possible in their own homes.



We provide rehabilitation in the patient’s place of residence, helping to maximise functional potential through tailored interventions, equipment provision and liaison with other services where required.



The role includes assessing functional ability and support needs, establishing meaningful rehabilitation goals, creating plans for support staff and identifying longer-term care requirements where appropriate.



We are seeking a motivated Physiotherapist to join our team and promote choice for our service users and their carers. You will be working alongside OT's, case managers and support workers and will develop overlapping skills with these professions.



In return, we'll give you the opportunity to develop skills and enhance your multidisciplinary working; we will provide you with regular supervision as well as the opportunity to develop your leadership skills.



We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs. Working arrangements include weekends and bank holidays on a rota basis.

  • Work as part of a wider MDT to achieve patient goals.

  • Communicate effectively with the wider MDT and case-manage services users as appropriate.

  • Delegate and supervise appropriate tasks and physiotherapy treatment plans to Rehabilitation Workers/ Physiotherapy Assistants/Technicians.

  • Be able to order equipment and ensure its safe and appropriate use, reporting any issues appropriately.

  • Facilitate referrals to other services based on your assessment to ensure holistic patient care.

  • Identify risks and act accordingly to minimise risk to self, service users and colleagues.

  • Actively participate in training; assisting in the delivery of training within the service and to other health professional as appropriate.

  • Actively participate in service development projects as required.
