Qualifications

• GCSE English or equivalent
• Level 3 in Health or Social Care or equivalent
• Good communication and IT skills
• Ability to work autonomously and as part of a multidisciplinary team

Responsibilities

• Support high quality rehabilitative palliative care for patients and carers
• Work across inpatient, outpatient, and community settings
• Manage own caseload and use mobility aids/equipment as required
• Maintain accurate patient notes and report changes in condition
• Participate in MDT meetings and follow PT policies
