Job summary

The Marie Curie service in South Liverpool requires an outreach-focused healthcare assistant to build relationships, support referrals, and signpost families to suitable palliative and end-of-life services. The role spans GP practices and community hubs, with autonomous caseload planning and collaboration with clinicians and other services.

Applicants should have Level 3 health or care qualifications and experience in palliative care.

Qualifications

• Level 3 health or care qualification required.
• Experience in palliative or end-of-life care desired.
• Experience delivering person-centred care preferred.

Responsibilities

• Carry out outreach and build relationships to reach people who need palliative care earlier.
• Support referrals and follow up on identified needs.
• Signpost people and families to appropriate services and help join up their care.
• Work with Clinical Nurse Specialists, primary care colleagues and community services across South Liverpool.
• Provide direct care where opportunities arise.
• Contribute to holistic assessment and care planning.
• Recognise and respond to changes in a person's needs.
• Contribute to symptom management within the scope of the role.
• Contribute to advance care planning.
• Hold compassionate conversations about dying, death and bereavement.
