Job summary

Marie Curie is recruiting two Clinical Nurse Specialists for a new palliative and end-of-life service across the South Liverpool primary care network. The work centres on finding people with palliative needs earlier, through outreach, referrals and advance care planning, and on specialist symptom management.

Each nurse will hold their own caseload while working across GP practices and community hubs, rather than from a single clinical base.

Qualifications

• Nursing degree with Level 6+ qualification
• Current NMC registration as a registered adult nurse
• Experience in specialist palliative care

Responsibilities

• Outreach and coordinate care across GP practices and community hubs
• Holistically assess people with complex needs
• Plan, implement and evaluate specialist treatments for symptom management
• Provide specialist palliative care advice and respond to changing needs using clinical judgement
• Support advance care planning and conversations about future wishes
• Coordinate care across organisations and advocate for person-centred care
• Manage own caseload and know when to collaborate or escalate
• Build relationships with patients, families and professionals across organisations
• Share expertise with colleagues and support multidisciplinary decision-making
• Contribute to learning, quality improvement and service improvement
• Provide expert guidance to other teams and act as a clinical role model
