Job summary

Qualifications

• Enhanced/Standard DBS check required.
• Full UK driving license essential.
• Post-qualification autonomous care experience desirable.
• Nursing role in community/home care environment.

Responsibilities

• Provide direct care to patients in their own homes, including palliative care support.
• Support the team with triage calls to patients, relatives and health professionals; participate in personal development reviews for junior staff.
• Ensure medical instructions are understood, adhered to and recorded; maintain equipment and vehicles used for visits.
