Job summary

Essex Partnership University NHS Foundation Trust is seeking an enthusiastic registered nurse to join the Thundersley/Benfleet palliative care team as a Band 6 post. You will hold a caseload of end-of-life patients and work within a multidisciplinary Palliative Care Team to deliver high-quality care and reduce inappropriate admissions through advanced care planning.

The role offers opportunity to develop within community palliative care, with support and supervision as part of an inclusive NHS

Job Overview

The community palliative care team are looking for an enthusiastic, motivated registered nurse to join Thundersley/Benfleet palliative care team as a band 6.

The post holder will hold a caseload of end of Life patients and work as a key member of the Palliative Care Team delivering care alongside nurses and therapists to end of life patients. The post holder will work to reduce all inappropriate admissions to secondary care through the use of advanced care planning and practical high quality end of life care.

Main duties of the job

To become part of the community palliative care team to provide holistic care for patients with life limiting conditions.
