Job summary

Essex Partnership University NHS Foundation Trust is seeking a Primary Care Mental Health Practitioner (Band 6) on a fixed-term 9-month maternity cover, based in Basildon. The role sits within Primary Care with joint work alongside VitaHealth Group to deliver a seamless community service.

You will triage patients, work within a multidisciplinary team, and ensure safe clinical practice aligned with NHS standards.

  • Experience of working in primary care/ community mental health.

Responsibilities

  • Provide a seamless service to aid the patient’s recovery and wellbeing in the community.
  • Work with the GP, Primary Care team and Consultant Psychiatrists to support patients in the most appropriate clinical environment.
  • Ensure safe clinical practice and maintain Care Quality Commission standards at all times.
  • Triage patients and facilitate them receiving the most appropriate support for their needs.
  • Deliver services that support clinical and practice governance.
  • Support the team’s continuous improvement in the quality of their service while safeguarding high standards of care.
  • Provide and undertake detailed physical health investigations such as BMI, blood pressure and blood tests as appropriate.

Primary Care Mental Health Practitioner

Base - Basildon and Brentwood

We are looking for a Primary Care Mental Health Practitioner to support the way forward in the delivery of Community Mental Health Services. The post will be hosted by Essex Partnership University NHS Foundation Trust (EPUT) with day today management within Primary Care, however the post holder will be working jointly with VitaHealth Group to provide a seamless service for this locality.

Main duties of the job
  • Provide a seamless service in order to aid the Patient’s recovery and Well‑being plans in the community.
  • Work in conjunction with the GP, Primary Care team and Consultant Psychiatrists to ensure that patients are supported in the most appropriate clinical practice environment from a Multi-disciplinary perspective.
  • Ensure safe Clinical Practice takes place, maintaining Care Quality Commission standards at all times and in line with the NHS Constitution.
  • Triage patients and facilitate them receiving the most appropriate support for their needs ensuring people are not passed around the system.
  • Deliver effective and safe services which support the key elements of
  • Clinical/Practice governance.
  • As the Primary Care model develops, the post holder will focus on supporting the team’s continuous improvement in the quality of their service, whilst safeguarding and maintaining a high standard of care by creating an environment in which excellence will flourish.
  • Provide and undertake detailed Physical Health investigations such BMI, Blood Pressure and request Blood Test as appropriate .
