Qualifications

Registered Occupational Therapist with a valid professional registration.
Strong written and spoken communication across diverse audiences.
Ability to manage a varied workload and meet short deadlines.
Experience operating under pressure with changing priorities.
Proactive and responsible with the ability to work across agencies.

Responsibilities

Assess adults in the community and plan individual care.
Manage risk and safeguarding responsibilities.
Involve moving and handling and reablement in line with policy.
Collaborate with health and social care colleagues to deliver practical solutions.
Provide community-based services from home visits to clinics.
