An exciting opportunity has arisen for a Community Mental Health Nurse or Preceptorship Nurse to join our established Older Persons Mental Health Community Team at Aerodrome House.
Your main responsibilities will include care coordination of service users, Duty Worker on rotation, initial and follow up assessments and supervision and mentorship of junior staff and students.
The post holder will be based at Aerodrome House and will need to work flexibly. You will also need to establish good working relationships with the local GPs, and other professionals.
The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues.
Due to the nature of this post in order to carry out home visits and attend patient meetings a car driver is essential with access to own car.

You will work within the Older People's Community Mental Health Team (CMHT) providing high-quality mental health nursing care to adults over the age of 65 with complex mental health or those under 65 with dementia including dementia, depression, psychosis, anxiety disorders, and other psychiatric conditions.
This role will primarily be delivered in a community setting, including patients' homes, residential and nursing care homes, nurse led clinics. You will work as part of a multi-disciplinary team (MDT) and collaborate with GPs, social care, pharmacy teams, voluntary agencies, and acute and crisis services to ensure safe and effective care planning.
