Dorset HealthCare University NHS Foundation Trust is seeking a Band 6 Community Mental Health Nurse for the Poole Community Mental Health Team. This role offers the chance to provide high-quality care to older persons with severe mental illnesses, within a supportive multidisciplinary team. Ideal candidates will be RMNs with experience in community care, bringing enthusiasm and a commitment to promoting well-being.
Poole Community Mental Health Team, Older Persons, is recruiting for a Band 6 Community Mental Health Nurse. This is an exciting opportunity to join our well-established integrated team.
If you are an RMN with experience working with individuals suffering from severe mental illness, whether inpatient or community-based, we would love to hear from you.
The service provides assessment, treatment, and support for individuals with organic and functional mental health illnesses. Experience working with older adults in inpatient or community settings is advantageous. The ideal candidate will be enthusiastic, self-motivated, and dedicated to delivering high-quality care to older persons, with a collaborative approach involving carers and other professionals. You will manage a defined caseload with support from the team leader and wider team.
Opportunities exist to develop clinical assessment skills, with risk assessment and management prioritized. We support ongoing professional development through additional learning opportunities.
The post requires a UK driving licence and personal transport.
We collaborate with various services across the Poole Locality to meet the mental health needs of the older adult population.
This role is challenging but supported by a well-established team. We seek someone reliable, committed, motivated, with excellent communication and organizational skills, capable of prioritizing caseloads alongside service demands.
The post involves working as part of an integrated multidisciplinary team including Psychiatrists, Nurses, Occupational Therapists, Psychologists, Social Services, Support Time Recovery Workers, and administration staff.
The primary responsibility is to act as a dedicated Care Coordinator, providing person-centered, holistic care to promote wellbeing and recovery.
Applications requiring current Skilled Worker sponsorship are ineligible.
Employment requires an Enhanced DBS check, which the Trust will cover. Applicants subscribed to the DBS update service can present a valid certificate instead of a new check.
